You grab the antacids from the medicine cabinet, chew a couple, wait for the relief — and probably don't think twice about it.

For most people, most of the time, that's fine. Heartburn is genuinely one of the most common digestive complaints in America, affecting roughly 60 million people at least once a month. Antacids are a $2 billion industry for a reason.

But here's what the antacid commercials don't tell you: that burning sensation in your chest can also be your heart. And the terrifying part is that even trained physicians sometimes get it wrong.

Two Conditions, One Very Confusing Symptom

Acid reflux — the technical term for heartburn — occurs when stomach acid escapes upward into the esophagus, causing irritation and that distinctive burning feeling. Cardiac events, including heart attacks and angina (reduced blood flow to the heart), can produce a sensation that is almost clinically identical: pressure, burning, or tightness in the chest, sometimes accompanied by a sour taste or a feeling that radiates upward.

The anatomical reason for this overlap is straightforward and kind of unsettling: the esophagus and the heart share nerve pathways. When either one is in distress, the brain receives pain signals through the same channels, and it doesn't always know — or tell you — which organ is actually the source.

"I've had patients describe what turned out to be a cardiac event using the exact same words they'd used to describe heartburn for years," says one emergency cardiologist. "They sat at home waiting for the antacids to kick in. That delay is sometimes the difference between a good outcome and a catastrophic one."

The Masking Problem: When Tums Becomes a Danger

Here's where the story takes a darker turn. Antacids work. They genuinely reduce the burning sensation — and that's a problem when the source of that sensation isn't your stomach.

In some documented cases, patients experiencing cardiac events have taken antacids, felt temporary relief (likely due to the placebo effect or a brief reduction in anxiety), and concluded that they were dealing with indigestion. Hours later, they arrived in emergency rooms with significantly more heart damage than they would have had if they'd sought care immediately.

The medical community has a term for this: "cardiac masquerade." It refers to the way heart symptoms can convincingly impersonate gastrointestinal complaints, and vice versa. It's not rare, it's not fringe medicine — it's a well-documented clinical challenge that plays out in emergency departments across the country every single day.

Long-term antacid use — particularly proton pump inhibitors (PPIs) like omeprazole, which are among the most commonly prescribed medications in the US — has also been associated in some research with increased cardiovascular risk, possibly through effects on the gut microbiome or magnesium absorption. The research is ongoing and not definitive, but it adds another layer to the conversation about reflexively medicating chest symptoms without understanding their source.

The Gut-Heart Connection Is Bigger Than You Think

The relationship between digestive health and cardiovascular health runs deeper than symptom overlap. The gut-heart axis — a term increasingly used in medical literature — describes the bidirectional communication between the gastrointestinal system and the heart, mediated by the vagus nerve, the microbiome, and inflammatory pathways.

Chronic acid reflux, when left unmanaged, can cause a condition called Barrett's esophagus — but it can also, through chronic inflammation and vagal nerve stimulation, contribute to heart rhythm abnormalities. Conversely, heart failure can cause fluid to back up into the digestive system, producing nausea, bloating, and symptoms that look a lot like gastrointestinal distress.

The two systems are not separate chapters in a medical textbook. They're in constant conversation, and what happens in one has real implications for the other.

So how do you know when your chest burn is a GI issue and when it's your heart asking for help? The honest answer is that you often can't know for certain without medical evaluation — and that's exactly the point. But there are some distinguishing features worth knowing:

By the time he arrives at the hospital, a significant portion of his heart muscle has been damaged by prolonged oxygen deprivation — damage that is permanent and that would have been substantially reduced or avoided with earlier intervention.

These aren't rare cases. They are common enough that emergency medicine physicians are trained to take any chest pain complaint seriously regardless of how confidently the patient insists it's "just acid reflux."

"The patients who scare me the most are the ones who are very sure it's heartburn," one ER physician noted. "Certainty is dangerous when you're talking about chest pain."

What Cardiologists Actually Want You to Do

If you experience chronic heartburn — meaning more than twice a week, or heartburn that requires regular antacid use — cardiologists and gastroenterologists both agree: see a doctor. Not to panic, but to establish a baseline understanding of what's happening in your body.

A proper evaluation can determine whether you're dealing with GERD, a more serious esophageal condition, or whether your symptoms warrant cardiovascular investigation. An electrocardiogram, a stress test, or simply a thorough clinical history can provide clarity that no amount of self-diagnosis can.

Stop treating your chest like a self-service problem. It's connected to the most important muscle in your body — and that muscle deserves a second opinion.
